This page is informational and does not replace a consultation.

Contraceptive implant in Marrakech: insertion, effects, removal

What is the implant?

It is a flexible rod about four centimetres long and two millimetres across, which continuously releases a progestogen. It prevents ovulation. Its effectiveness is over 99%; it works for three years and its effect disappears after removal. It contains no oestrogen and does not protect against sexually transmitted infections.

Who is it for, and who not?

The implant suits most women, including adolescents, women without children, those who are breastfeeding or who cannot take oestrogen (migraine with aura, history of phlebitis). It is not advised in case of pregnancy, breast cancer or another hormone-sensitive cancer, serious liver disease, unexplained genital bleeding or ongoing thrombosis.

Some medicines (certain anti-epileptics, rifampicin, St John's wort) reduce its effectiveness: tell us about all your treatments.

Before insertion

A consultation checks that the implant suits you and a pregnancy test is done if needed. Insertion is possible at any time if pregnancy is ruled out. Inserted during the first days of the cycle, it protects immediately; otherwise a condom is advised for the first week. It is compatible with breastfeeding.

How does insertion go?

It is done on the inner side of the non-dominant arm, above the elbow crease. After disinfection, a small local anaesthetic injection is given, then the rod is slid under the skin with a sterile single-use applicator. The procedure lasts a few minutes. A small dressing is applied, and the doctor shows you how to feel the rod under the skin.

After insertion

A little bruising or tenderness of the arm is possible for a few days. Keep the dressing on for a few days and avoid knocking that spot. You should be able to feel the rod; if you can no longer do so, consult.

Effects on periods and other effects

The most common effect concerns bleeding: irregular periods, sometimes frequent or prolonged, or on the contrary no periods. This pattern cannot be predicted before insertion. Other effects are possible: acne, headaches, breast tenderness, mood changes. Some women report weight gain, without the implant being proven to be a direct cause.

Troublesome bleeding is the first reason for removal: it is better to talk about it beforehand, and to consult if needed, as solutions exist.

Rare complications

They are uncommon: bruising or infection at the insertion site, a scar, movement of the rod (very rare), difficulty with removal if the implant is badly placed. Careful insertion and checking that it is there limit the risk.

Removal or replacement

The implant is removed after three years, or earlier if you wish. After local anaesthetic, a very small incision lets it be taken out in a few minutes. A new implant can be inserted straight away. After removal, fertility returns quickly.
